Requesting a
predetermination is an
optional step and is intended for individuals who have not yet submitted a formal application for licensure.
You are not required to obtain one before submitting a formal license application. However, a formal licensing application is still required for licensure.
If you have already submitted an application, please
DO NOT submit a request for a predetermination.
The application fee is
$25 in addition to
a transaction fee of
$4.00 for e-check
or
3% + 2.00 for credit/debit card.
