  • Vaults or Outer Burial Containers - If it is the Preneed Seller’s practice to install vaults or outer burial containers before need, then the Preneed Contract Purchaser must sign this disclosure at the time the preneed contract is executed.
  • Life Insurance or Annuity Purchased By Trust - If the Preneed Seller has a Merchandise & Services Trust in which life insurance or annuities is a permitted investment, then the Preneed Contract Beneficiary must sign this disclosure prior to the trust’s purchase of life insurance or an annuity on the life of the Preneed Contract Beneficiary.
