Vaults or Outer Burial Containers - If it is the Preneed Seller’s practice to install vaults or outer burial containers before need, then the Preneed Contract
Purchaser must sign this disclosure at the time the preneed contract is executed.
Life Insurance or Annuity Purchased By Trust - If the Preneed Seller has a Merchandise & Services Trust in which life insurance or annuities is a permitted investment,
then the Preneed Contract Beneficiary must sign this disclosure prior to the trust’s purchase of life insurance or an annuity
on the life of the Preneed Contract Beneficiary.
